No:2026/009Clinical Incident ManagerGrade VIIFull Time Permanent ContractThe Clinical Risk and Patient Safety Department is fundamental to the provision of high quality, safe clinical services and systems in the Rotunda by ensuring that patient safety is always a priority at the core of service provision.Management and review of clinical incidents and near misses are key components of healthcare management. This will involve providing advice, guidance and support to staff and undertaking practical activities such as risk assessment, incident management and reviews. The focus of the department is to improve performance in the delivery of patient care throughout the hospital underpinned by the four core principles outlined in the Hospital Strategic Plan We are currently looking to hire a Grade VII Clinical Incident Manager in our Clinical Risk and Patient Safety Department here in the Rotunda Hospital. The successful candidate will be working alongside a team of dedicated professionals under supervision of the Head of Clinical Risk Medico legal department. The successful candidate will provide a high quality clinical risk and safety management service to the Rotunda Hospital ensuring patient safety is always a priority at the core of providing services. This will involve undertaking practical activities such as incident assessment and management of incident review reports. Providing advice, guidance and support to staff within the department and clinical staff who have been involved in an adverse clinical incident. The post holder will actively promote collaboration across professional boundaries with the aim of developing and embedding an open and fair culture and ensuring that lessons are learnt from incidents and claims and recommendations are implemented.A key activity to promoting this culture will be through the delivery of clinical incident management training to all staff.Based in the City Centre and on the grounds of the oldest continuously operating maternity hospital in the world, you will have the opportunity to work with colleagues across all professional disciplines and become part of the Rotunda family.Qualifications And ExperienceEssential:Minimum 2 years work experience in Risk Management/ Legal Studies/Patient SafetyClinical qualification e.g. Midwifery/Nursing, Allied Health Professional.Post graduate education in Risk Management/ Legal Studies/Patient SafetyExcellent communication skills, report writing skills and the ability to manage projects and work to deadlines.Excellent IT Skills including Microsoft Office and the ability to collate qualitative and quantitative information into reports.Experience in risk assessment.Experience in training and developing staffDesirableExperience in incident management, systems analysis review and report writingExperience with change management and committee work.Leading and managing staffUnderstanding HSE/HIQA clinical governance and controls.Understanding of current maternity risk issuesPrincipal Duties And ResponsibilitiesLead and managethe review of all clinical incidents ensuringthey are appropriately categorised, assessed and followed-up in accordance with local and national guidance( approximately 300 incidents per month)Leadthe review and investigation of Serious Reportable Events and Serious Incidents and ensure the timely initiation and completion of System Analysis Reviews, in accordance with the HSE's Incident Management Framework and Systems Analysis Guidance for Services.Manage staff within the department including Grade 6 Clinical RiskCo-ordinator and Grade 5 Clinical Risk Administrative staff ensuring their skills and expertise are developed effectively and efficientlyManage theeducation and trainingfor the above staff to ensure their skills and expertise are used effectively and efficiently.Make decisions and solve problems in a timely manner to includeescalationto senior management and or external organisations.Where appropriate inform clinical leads of decisions that have implications for their area.Manage the development of policies and procedures and ensure consistent adherence to procedures and current standards within Clinical Risk with particular focus on incident managementManage resources, effectively handle multiple projects concurrently, structuring and organising own workload and that of others effectively.Produce reports for Departmental and Committee meetings including Quality & Safety and the DNE acute HospitalsWomen & Infants Senior Incident Management Forum.Manage and prepare for Coroners Inquest by providing guidance, support and assistance to staff throughout the incident management process, up to and including attendance in Court or at a Coroners Inquest.Represent the department at hospital committee meetings and EMT meetingsLead in project working groups and represent the Rotunda on committees as required eg HSE Dublin and North East Acute Hospitals, 3 Maternity hospitals groupMaintain a positive relationship with all clinical staff to ensure effective communication of all issues relating to clinical risk.Work collaboratively with Quality Patient Safety department to promote and support the implementation of open disclosure through staff education, training and support.Identify the most appropriate staff to implement recommendations arising from completed Systems Analysis Reviews and monitor the progress of implementation.Assist the Clinical Risk and Safety Manager with the claims management function and Inquest requirements.Ensure the timely release of all relevant documentation by working closely with the Information Governance Manager and other staff.Provide guidance, support and assistance to staff throughout the incident management process, up to and including attendance in Court or at a Coroners Inquest.Deputise for the Clinical Risk and Safety Manager, as required.Note: These responsibilities and accountabilities are a guide only and are subject to addition, review and amendment to meet the changing needs of the Hospital.All Informal Enquires In Relation To The Above Should Be Directed To Ms. Siobhan Enright, Clinical Risk & Patient Safety Manager.
