Job Description
We are seeking an experienced and dedicated Community Health Care Professional to provide high-quality, person-centred care to children with life-limiting conditions and their families in the family home/community environment. Key Responsibilities:
* Develop and implement individualised care plans that meet the unique needs of each child and their family.
* Provide high-quality, compassionate care to children with life-limiting conditions and their families.
* Work collaboratively as part of an interdisciplinary team to achieve positive outcomes for patients and their families.
* Contribute to the development and implementation of policies and procedures that promote excellence in patient care.
Requirements:
* Be fully registered with the relevant healthcare authority.
* Have post-registration experience in a paediatric setting, preferably an acute setting.
* Have a working knowledge of information technology.
* Be able to work independently and as part of an interdisciplinary team.
