Responsibilities
The Clinical Nurse Care Coordinator collaborates with clinic team members to provide evidence-based, patient-centered care, helping patients stay up to date on their care plans and related health goals.
This role focuses on high-risk, medically complex patients, those on specific disease registries, patients with chronic disabilities, and those in care transitions, acting as a liaison with families and other agencies to facilitate comprehensive, whole-person care.
The RN stays current on standards and assists with programs and processes.
Qualifications
* Associate Degree in Nursing from a CCNE or ACEN accredited program.
* Current Ohio RN license.
* CPR/BLS & ACLS certification.
* Experience with triage procedures in clinics, physician offices, or urgent care settings.
* Ability to maintain quality, safety, and infection control standards.
* Knowledge of medication administration procedures.
* Ability to educate and communicate effectively with patients and families.
* Strong written and verbal communication skills.
* High interpersonal skills for handling sensitive situations.
* Poise, tact, and diplomacy.
* Analytical and critical thinking skills for data interpretation and problem-solving.
Pay Range
USD $31.11 - USD $46.66 per hour.
