GSW Solutions are recruiting on behalf of their client for a Social Care Team Leader.
This role is an integral part of the support team, and is responsible for meeting the needs of the individuals we support within the service.Key AccountabilitiesCoordinate Transitions & Assessments: Oversee the safe transition of individuals into services and coordinate ongoing assessments to tailor support plans effectively.Develop & Implement Support Plans: Design and deliver individual and group support programs to enhance independence and well-being.Foster Communication & Advocacy: Liaise with families, multidisciplinary teams, and community groups to promote the needs and rights of those supported.Provide Leadership & Supervision: Lead and manage Support Workers, including performance oversight and participation in HR procedures.Ensure Compliance & Best Practices: Uphold standards in medication management, record-keeping, and regulatory compliance while promoting dignity, choice, and respect.Requirements of the RoleTake part in the company's performance appraisal and staff review processes.Consistently demonstrate and uphold the organization's core values, behaviours, and competencies.Actively participate in both formal and informal supervision sessions as needed.2 years experience in a healthcare or social care setting with minimum of QQI Level 5.Full Irish Driver's License Salary€37,900.00 - €49,800.00 per year
