Role Overview:
We are seeking a dedicated and compassionate Social Worker to join our multidisciplinary team. As a key member of the team, you will work closely with adults with intellectual disabilities and their families to provide person-centred support and promote independence and community participation.

This is an exciting opportunity for a Social Worker to make a meaningful difference in the lives of individuals with intellectual disabilities. You will be responsible for assessing needs, developing care plans, safeguarding vulnerable adults, and advocating for appropriate supports and services.
Main Responsibilities:
* Conduct thorough psychosocial assessments and develop individualised care plans in collaboration with service users, families, and carers.
* Support adults with intellectual disabilities in navigating life transitions, such as independent living, employment, and community integration.
* Provide emotional and practical support to families and caregivers, ensuring they are equipped to meet the needs of their loved ones.
* Promote and advocate for the rights and welfare of service users in accordance with relevant legislation and guidelines.
* Participate in safeguarding procedures, reporting concerns of abuse or neglect and following up as per national policy.
* Work collaboratively with internal and external stakeholders including disability support teams, primary care, day services, and residential settings.
* Maintain accurate, timely, and professional documentation and case notes in accordance with best practice and data protection standards.
* Contribute to service planning and development by identifying gaps in services and advocating for improvements.
* Participate in team meetings, supervision, and ongoing professional development.
