Occupational Therapy Assistant - ICPOP St Marys Hospital Campus:
The
Integrated Care Programme for Older Persons (ICPOP)
is an HSE service for older people who have complex medical needs or frailty.
The main goal is to provide coordinated care at home or in the community, helping older people live well independently and avoid unnecessary hospital admissions and emergency department presentations.
Key features of the service include:
* A Multidisciplinary Team: The team is led by a consultant and includes specialist nurses, physiotherapists, occupational therapists, and other health professionals.
* Comprehensive Assessment: The team conducts a full review of the patient's health, mobility, home environment, and social supports.
* A Personal Care Plan: Based on this assessment, the team works with the patient and their family to create a single, coordinated care plan.
* Referral: Access to the service is typically through a referral from a GP or Frailty Intervention and Therapy Team (FITT) based in the Emergency Department).
The ICPOP service is organised into different teams, each aligned with an acute hospital and covering a specific community area.
The ICPOP teams for Dublin North West and City cover 7 CHN's. Blanchardstown, Blakestown, Cabra, Finglas, Ballymun, Clontarf and North Inner City
