As a key member of our team, you will work closely with Physiotherapists, Nurses, Social Workers, Speech and Language Therapists, and GPs to provide coordinated service delivery and support client participation in community activities.

About the Role

* Provide leadership and guidance to junior therapists, promoting best practice and continuous professional development.

* Liaise with external services, voluntary organisations, and hospital teams to ensure continuity of care and coordinated service delivery.
* Maintain accurate and timely clinical records, reports, and care plans in line with HSE and CORU standards.

Key Responsibilities

1. Develop and maintain relationships with clients, families, and carers to promote safe and effective daily living at home.
2. Conduct home visits to assess clients' living environments, recommend adaptations, and provide interventions to maximise independence.
3. Deliver rehabilitation programmes aimed at falls prevention, frailty management, and functional recovery following illness or injury.
4. Provide education and training to clients, families, and carers to support safe and effective daily living at home.

Requirements

* Master's degree in Occupational Therapy or related field.
* Minimum 3 years' experience as an Occupational Therapist in adult rehabilitation, with a focus on frailty and community-based care.
* Strong leadership and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
