You will be responsible for co-ordinating activity and resources, facilitating communication among healthcare professionals, and providing comprehensive education and support to patients with diabetes.
You will work closely with other healthcare professionals to develop service plans that meet the needs of patients with complex conditions. This may involve working within existing guidelines or developing new protocols where necessary.
