About the Role:
The Programme Facilitator provides critical support to service users, facilitating both internal and external programmes within the service.
This role involves providing vehicle escort support for service users travelling to and from the centre and their homes, as well as supporting service users with personal care where required.
As a key worker, you will conduct key worker meetings and updates, and assist with Person-Centred Planning (PCP) as directed by the centre supervisor.
We are seeking a highly skilled individual who has a minimum QQI Level 5 in Social Care or equivalent, experience supporting adults with an intellectual disability and people enduring mental health issues, and programme facilitation skills.
Your responsibilities will include being a key worker to service users to facilitate the Person-Centred Planning process, identifying goals and supports required by individual service users and maintaining records.
You will also be responsible for developing and delivering person-centred programmes to meet the needs of service users as identified through Person Centred Planning.
In addition to these duties, you may be required to provide administrative support to the team as needed.
