About the Role:
As a Senior Physiotherapist in this Primary care Community Team, you will play a vital role in assessing, treating, and supporting patients in the community. Your work will help individuals regain mobility, maintain independence, and improve their overall quality of life.
Key Responsibilities:
* Provide comprehensive physiotherapy assessments and treatments for individuals of all ages with a range of conditions, including musculoskeletal, neurological, and age-related conditions.
* Work collaboratively with the Senior Occupational Therapist, a second Senior Physiotherapist, and a Physiotherapy Assistant to deliver patient-centred care.
* Develop and implement individualised treatment plans that address each patient's specific needs and rehabilitation goals.
* Facilitate early hospital discharge by delivering rehabilitation and reablement interventions within the community.
* Liaise with GPs, nurses, and other healthcare professionals to ensure seamless and coordinated care.
* Participate in regular supervision, peer learning, and professional development to enhance clinical expertise.
* Contribute to service development and quality improvement initiatives to optimise patient outcomes and enhance service delivery.

Requirements:
* BSc in Physiotherapy.
* CORU registration as a Physiotherapist.
* Minimum of three years of post-graduate Physiotherapy experience, with at least one year at Senior level (preferably in a community or rehabilitation setting).
* Strong experience in, Rehabilitation, falls prevention, Musculoskeletal, and/or Neurological Physiotherapy.
* Ability to work independently and as part of a team, demonstrating leadership and initiative.
* Excellent communication, interpersonal, and problem-solving skills.
* Full clean driver's license and access to a car for community visits.
