TCP Homecare are a leading provider of Homecare Nursing and Pharmacy Services on a national scale.
An exciting opportunity has arisen for the role of Pharmacy Patient Coordinator.
We are seeking an experienced pharmacy technician or dispensing assistant with strong administration skills for the post.
This is a permanent, full time Monday to Friday position.
There are no late nights or weekends.
Our patients are based nationwide.
The ideal candidate will have attention to detail, be a self-starter and exhibit follow through on tasks.
Pharmacy technician or dispensing assistant qualification a distinct advantage.
Experience in either a hospital or community position is highly desirable.
The role is an office based administrative role, working within a wider team.
The patient coordinator is responsible for coordinating the medication deliveries to patients nationwide and looking after patient requirements.
We are looking for a hardworking and enthusiastic individual who will display flexibility and ability to adapt to change, to join our pharmacy team.
This role provides the opportunity to gain experience in a quality environment, alongside unique pharmacy experience in rare treatment areas.
You will be part of a professional, quality focused team, who continually strive to provide the highest clinical care to patients.
Full training is provided and there is excellent support staff in place.
Key Qualifications & Competencies: Experienced administrator with demonstrable computer skills Qualified pharmacy technician or dispensing assistant a distinct advantage Strict attention to detail and a quality focus Hard working and enthusiastic Flexible and willing to learn, working side by side with pharmacist and customer service team The ability to work in a self-directive manner, while also being receptive to instruction Excellent time management and prioritisation skills, works well to deadlines A good team player Excellent communication skills with both patients and professional staff, excellent written and verbal English required Relevant hospital or community pharmacy experience Job Scope: The pharmacy patient coordinator is responsible for looking after patient needs while working with the wider pharmacy and logistics team.
Key Responsibilities: Complete the calls to patients in a time effective manner in advance of the delivery date.
Conduct regular stock takes with the patients, encourage stock rotations in the home by expiry and check the expiry dates on bleed dose stocks.
Work with the patients in as far as possible to minimise waste, and liaise with pharmacy and hospital as appropriate to assist with same.
Process the patient orders for drugs and ancillaries in line with the prescription.
Work closely with the pharmacy team to notify the team of any prescriptions required in advance of delivery.
Work within the guidelines and policies of the Pharmaceutical Society of Ireland as applicable.
Assist the chief pharmacist and stock controller as appropriate with stock management, help to notify the chief pharmacist if stocks are low, or if special orders are required.
Maintain the schedule for routine delivery routes and assist with route planning.
Schedule ad hoc/off route deliveries to patients as required.
Work with the warehouse team lead and/or logistics manager to manage emergency/on call deliveries where Contribute to the TCP system for managing customer complaints and investigating incidents related to the service.
Contribute to error management, near miss reporting, and associated corrective and preventative actions.
Manage patient related calls via the dedicated phone line during office hours and attend to emergency calls in a appropriate timeframe.
Review the stocks in the hospital labs as per monthly report and work to minimise wastage.
Act as a point of contact for the blood laboratories in hospitals nationwide.
Maintain customer confidentiality at all times.
Represent self in a professional and ethical manner at all times, complying with all applicable company policy and procedures.
Be responsible for fostering a progressive working environment within the pharmacy department and for professional and courteous communications to others.
Form part of an engaged team with a can do attitude, exhibiting flexibility as required.
