Discharge Co-Ordinator - Rehabilitation Service/Older Persons Rehabilitation

The post holder will report to the Assistant Director of Nursing Bed Management and is professionally accountable to the Director of Nursing Mercy University Hospital.
Details of Service:
Blarney Rehabilitation Unit is a newly developed purpose-built post-acute care rehabilitation unit. It is made up of 50 beds located off site from the MUH. It remains under the governance of the MUH. The Rehabilitation Services program delivers specialist post-acute care, focusing on patient-centered reablement to help patients achieve optimal functionality and independence following illness or injury. This service emphasizes seamless transitions of care between hospital and home, integrating multidisciplinary expertise to support patient recovery.
Responsibilities - The post holder will be responsible for;
* Conduct ongoing assessments of patients' clinical and social needs to determine timely discharge readiness.
* Develop, implement, and monitor individualized discharge plans in collaboration with consultants, CNM2, and the multidisciplinary team.
* Liaise with community nursing, primary care, social work, and allied health professionals to arrange postdischarge supports and services.
* Chair or participate in regular multidisciplinary discharge planning meetings.
* Act as the key contact for Ward managers, therapists, medical staff, and bedmanagement personnel to resolve discharge barriers.
* Record and track discharge metrics (e.g., average length of stay, delayed transfers of care) using hospital IT systems.
* Provide guidance and education to nursing and therapy staff on bestpractice discharge processes and documentation standards.

* Design and undertake clinical audits of discharge activities against agreed standards and guidelines.
* Lead quality improvement initiatives to enhance patient flow, reduce readmissions, and optimise resource use.
* Uphold NMBI Code of Conduct and organisational policies at all times.
* Support staffing decisions related to discharge coordination, including roster planning and crosscover arrangements.
Eligibility Criteria:
* Be registered in the General Division of the Live Register of Nurses by Nursing and Midwifery Board of Ireland (NMBI)
* Have a proven track record in General nurse management
* Have at least 5 years post registration recent experience, with 2 years' experience in the relevant area.
* Have computer and analytical skills
* Management course desirable
* Post registration qualification in Nursing and or Management desirable.
Experience
* Previous experience of change management and project delivery
* Experience of leading quality improvement projects
* Possess excellent interpersonal skills and be capable of communicating effectively and efficiently with a wide variety of departments and individuals, including patients and co-workers.
* Have the ability to deal with challenging situations and the ability to proactively influence outcomes
* Possess good decision-making skills and a capacity for analytical thinking and problem solving with a good knowledge of clinical nursing
* Have the ability to provide motivation and team-leadership to all employees within your remit within the clinical areas
* Provide evidence of ongoing professional development
