Occupational Therapy Role
We are seeking an experienced and skilled Occupational Therapist to join our team, working with adults who require rehabilitation and support.
* The successful candidate will deliver evidence-based interventions focused on maximising independence, functional rehabilitation, falls prevention, cognitive assessment, and environmental adaptation.
* Key responsibilities include conducting assessments in client's homes, developing tailored rehabilitation programmes, providing client-focused treatment, and creating goal-focused care plans.
* You will work collaboratively as part of a multidisciplinary Primary Care Team, including Physiotherapists, Public Health Nurses, GPs, Social Workers, SLTs, and other allied health professionals.
* Liaise with hospitals, community organisations, voluntary agencies, and other stakeholders to ensure coordinated care.
* Provide clinical leadership, supervision, and support to junior colleagues, therapy assistants, and students.
