We are seeking a dedicated and compassionate individual to join our team as a Service Coordinator.
This is an exciting opportunity for someone who wants to make a positive difference in the lives of people with intellectual disabilities, acquired brain injuries, and autism.
As a Service Coordinator, you will work collaboratively with a multi-disciplinary team to provide person-centered support to residents.
Key Responsibilities
* To assist persons with a disability with residential, training, and day service activities.
* To maintain independence and/or improve skills that will make them more independent.
* To work collaboratively as part of a multi-disciplinary team in service delivery to residents.
Requirements
* A qualification at Level 5 on the National Framework of Qualifications in Social Care, Social Work, Youth Work, Teaching, Nursing, Childcare with Disability modules.
* Three years experience working in a residential, respite or day service setting with people with an intellectual disability.
