About the Role As a Senior Physiotherapist in this Primary care Community Team, you will play a vital role in assessing, treating, and supporting patients in the community. Your work will help individuals regain mobility, maintain independence, and improve their overall quality of life. Key Responsibilities: Provide comprehensive physiotherapy assessments and treatments for individuals of all ages with a range of conditions, including musculoskeletal, neurological, and age-related conditions. Work collaboratively with the Senior Occupational Therapist, a second Senior Physiotherapist, and a Physiotherapy Assistant to deliver patient-centred care. Develop and implement individualised treatment plans that address each patient's specific needs and rehabilitation goals. Facilitate early hospital discharge by delivering rehabilitation and reablement interventions within the community. Liaise with GPs, nurses, and other healthcare professionals to ensure seamless and coordinated care. Participate in regular supervision, peer learning, and professional development to enhance clinical expertise. Contribute to service development and quality improvement initiatives to optimise patient outcomes and enhance service delivery. Minimum of three years of post-graduate Physiotherapy experience, with at least one year at Senior level (preferably in a community or rehabilitation setting). Strong experience in, Rehabilitation, falls prevention, Musculoskeletal, and/or Neurological Physiotherapy. Ability to work independently and as part of a team, demonstrating leadership and initiative. Excellent communication, interpersonal, and problem-solving skills. Full clean driver's license and access to a car for community visits.
