3. One permanent, full-time vacancy in Vergemount Community Mental Health Team, Co. Dublin
A panel may be formed from this campaign for Clinical Nurse Specialist (Mental Health), which may be used to fill current and future, permanent and specified purpose vacancies of full or part-time duration.
Purpose of the Post:
The purpose of this Community Mental Health Nurse (Mental Health) post is to:
* Deliver care in line with the five core concepts of the role as set out in the Framework for the Establishment of Community Mental Health Nurse Posts, 4th edition, NCNM 2008.
* Work as a key member of the multidisciplinary Mental Health team, providing a patient/client-centred, quality service. This includes assessment, planning, delivery, and evaluation of care for patients/clients and their families.
* Provide a clinical focus that encompasses assessment, planning, delivery, and evaluation of care.
* Include weekend shifts to ensure access to a 7-day service for those requiring support.
* Deliver effective, recovery-oriented, high-quality, team-based, integrated care for individuals with mental health problems.
* Provide assessment and support through appropriate treatment and recovery interventions to reduce the need for hospitalisation, including domiciliary visits, crisis resolution, family support, and liaison with other services.
* Manage a caseload of service users aged 18 and over, from General Adult Mental Health Teams, with acute mental health needs or long-term mental illness.
