Job Description:
We are seeking a skilled and compassionate individual to fill the role of Relief Programmes Facilitator. As a key member of our team, you will play a vital part in supporting adults with intellectual disabilities and mental health issues.
About the Role:
This is a 12-month contract, working variable hours per week at our Lifford Resource Centre. You will report to the Community Service Manager through the Programmes Supervisor and be responsible for facilitating service users in their choices, developing programmes within a person-centred model of service delivery.
Your Responsibilities:
* To be a key worker to service users, facilitating the Person Centred Planning process, identifying goals and supports required by individual service users, and maintaining records as required.
* To develop and deliver person-centred programmes to meet the needs of service users, as identified through Person Centred Planning, and to maintain the required records.
* To support individuals and groups in accessing their community to meet identified needs, facilitating activities and programmes, and any other reasonable duties as deemed appropriate by the Programmes Supervisor/Community Services Manager.
* To work on your own initiative as a lone worker, liaising with relevant team members to ensure that service users receive the highest standard of service, communicating with external agencies and families, and maintaining relevant records.
Requirements:
We are looking for an individual with a minimum QQI Level 5 in Social Care or equivalent, and a third-level graduate in Health & Social Care or equivalent would be desirable. Experience supporting adults with intellectual disabilities and people enduring mental health issues is essential.
