Job Title: Care Manager
We are seeking an experienced Care Manager to join our team. As a key member of our organization, you will play a vital role in developing and implementing comprehensive care plans, managing teams, and collaborating with healthcare providers, patients, and families.
Key Responsibilities:
* Develop and implement individualized care plans that cater to the unique needs of patients and their families.
* Manage and lead teams of healthcare professionals, providing guidance and support as needed.
* Collaborate with healthcare providers, patients, and families to ensure seamless communication and coordination of care.
* Mentor and develop staff members, fostering a culture of excellence and continuous learning.
* Ensure compliance with regulatory requirements and organizational policies.
Requirements:
* A minimum of two years' experience as a Registered Nurse in a relevant healthcare setting.
* A nursing qualification and registration on the live register of NMBI for at least three years (RGN desirable, other disciplines with relevant experience considered).
* Excellent communication and interpersonal skills, both verbal and written.
* Strong analytical and problem-solving skills, with the ability to think critically and creatively.
* Proficiency in computer administration and software applications.
* Experience in staff management and leadership development.
* Ability to maintain quality standards and ensure health and safety compliance.
