About the Role
As a key member of our team, you will facilitate service users in their choices, developing programmes within the context of a person-centred model of service delivery. Your primary focus will be on providing high-quality support to ensure the needs of service users are met.
Key Responsibilities
* Develop and deliver person-centred programmes to meet the needs of service users, maintaining accurate records.
* Act as a key worker to service users, facilitating the Person Centred Planning process, identifying goals and supports required by individual service users, and maintaining records.
* Support individuals and groups in accessing their community, facilitating activities and programmes, and performing other reasonable duties as deemed necessary.
Requirements
To succeed in this role, you will need:
* A minimum QQI Level 5 in Social Care or equivalent qualification.
* Good knowledge of New Directions and community-based programmes.
* Knowledge and understanding of intellectual disabilities and mental health issues.
* At least one year's experience working in a relevant setting, with experience of facilitating and implementing person-centred plans.
