Learn more about how we're raising the bar in new ways in just 2 minutes at: RESPONSIBILITIES - as RN, Nurse Home Care Case Manager, you oversee the nursing standards for the Caring for Family of Companies.
You're responsible for the highest quality of case management and care for Clients – as well as providing high caliber Caregiver training and professional development.
You maintain strict compliance with established protocols and the In Home Care licensing standards.
This position reports to the Director of Nursing Services, and collaborates with an entire team of Nurses for ongoing support.
Oversee nursing services, including medication administration programEnsure standardization and adherence of processMaintain Client confidentialityReview medication administration and medication error reports to maintain safety and compliance protocolsReview medication orders to confirm Client needs and contradictions, collaborating with providers for adjustments when neededConduct nursing and medication evaluation and assessment visits, addressing Client concernsTrain Caregivers on medication administration and evaluate competencyTrain Caregivers on Nurse-delegated duties, ensuring competency and ability to deliver careTravel to regional branches to fill staffing gaps, perform nursing assessments, and train on medication administration – as neededROLE QUALIFICATIONS Available for Tuesday staff trainings.Available 8-10 hours a week.Have a high value for innovation, streamlined processes and efficiencyDemonstrate a pro-active and solution-oriented mindsetHave excellent written and oral communication articulation in EnglishDemonstrate accuracy and attention to detail, maintaining accurate records and documentationPossess intuitive, strategic, and organized project management skillsHave excellent professional collaboration skills and team mindsetHave a results-driven, task-oriented approachUtilize excellent time management skills, meeting deadlines, and handling multiple tasks in a fast-paced environmentDemonstrate strong interpersonal, discretion, and judgment skillsHave proficiency with technology and electronic devicesDemonstrate sense of warmth, welcome, and professionalismDemonstrate strong commitment to complianceKnowledge of best practices for handling PHI in compliance with HIPAA regulationsCOMPREHENSIVE WELLNESS PORTFOLIO - In your role with Caring for Family of Companies, you're responsible for upholding, executing, and supporting our mission.
