Support Worker Childrens disabilities The Support Worker will work as part of an interdisciplinary team to deliver a quality service to children and their families who are attending the service. The support worker will report to the Childrens Disability Network Manager. Responsibilities of Support Worker Assist with review of clinical reports from private providers, documenting the outcomes and recommendations in a standardised template under the supervision of the Children's Disability Network Manager. Assist with transfer / referral to other services where appropriate under the supervision of the Children's Disability Network Manager and clinical team. Assist with the discharge of clients from the CDNT under the supervision of the Children's Disability Network Manager and clinical team. Assist with file reviews and preparation of clinical note summaries under the supervision of the Children's Disability Network Manager and clinical team. Assist in the administration and day to day operation of the service as needs arise. Research and prepare resources for universal family support and strategies. Participate in team meetings and report to the Children's Disability Network Manager on matters affecting the delivery of service. Maintain a high standard of documentation, including service user files in accordance with local guidelines, the principles of confidentiality, the Freedom of Information (FOI) and GDPR Acts. Contribute to the development and implementation of information sharing protocols and audit systems. Contribute to the ongoing development of the service in keeping with good practice and HSE objectives. Maintain professional standards in relation to consent, confidentiality, ethics and legislation. Observe all rules relating to Health and Safety and Conduct at Work and use any equipment provided in a safe and responsible manner. Requirements of Support Worker QQI Level 5 Award in Health Care Support / Social Care / Special Needs Assisting (or equivalent). Experience supporting children or individuals with disabilities.
