Programme Facilitator Job Opportunity
At our organization, we are seeking a dedicated and compassionate Programme Facilitator to join our team. As a key worker, you will be responsible for facilitating the Person-Centred Planning process and supporting individuals with their goals and aspirations.
About the Role:
* Develop and deliver person-centred programmes that meet the needs of service users as identified through Person Centred Planning.
* Support individuals in accessing their community to meet identified needs by facilitating activities and programmes.
* Liaise with relevant team members to ensure seamless delivery of services.
Required Skills and Qualifications:
The ideal candidate will possess a minimum QQI Level 5 in Social Care or equivalent. A third-level graduate in Health & Social Care or equivalent is desirable. Experience supporting adults with intellectual disabilities and mental health issues is essential. Additionally, programme facilitation skills an advantage.
