We have a part time band 3 Community Healthcare Assistant position in our Ashbourne Community Nursing Team, based at St. Oswald's Hospital.
We cover a large geographical area and also work closely with our other planned and rapid teams in the Derbyshire Dales.
The job would involve working x3 6hr days over the week and include pro rata weekend working.
This is a great opportunity to work within an experienced team to improve knowledge and skills and be supported to complete competencies and develop.
Responsibilities
To provide holistic care to patients in their own homes, under the supervision of the appropriate registered professional.
To report and take appropriate action regarding any change in patient condition or circumstance and communicate verbal and written information to all relevant integrated care team members.
To maintain and record accurate health records electronically, in line with DCHS policies.
To participate as a team member in a wider integrated team, in continuous learning to develop your own knowledge and skills.
To have a level of understanding with regards to the importance of safeguarding.
Respect the dignity of individuals.
Obtain consent when involving the patient, their carers/families/relatives in shared decision making.
To undertake all aspects of clinical duties and procedures which are documented using SystmOne.
To work within your own skill set to deliver the following: Personal care, Urinary management, Bowel management, Continence management, End of life care, Clinical observations, Pressure area care, Moving and handling and Medication administrations.

Able to work over a 7-day week, including weekends and bank holidays (pro-rata)
