About the Role

The successful candidate will have expertise in managing diverse physical, functional, and psychosocial needs, as well as experience in conducting comprehensive assessments and developing collaborative intervention plans.
* Develop treatment plans tailored to individual needs, incorporating self-management strategies, equipment provision, environmental adaptations, and rehabilitation programs.
* Collaborate with multidisciplinary teams to provide integrated care, including GPs, nurses, occupational therapists, speech and language therapists, social workers, and other professionals.
* Contribute to health promotion and prevention initiatives for adults, enhancing their quality of life and overall well-being.
Key Responsibilities
* Manage a diverse adult caseload, delivering therapy interventions in clients homes, clinics, and community settings.
* Deliver high-quality, evidence-based interventions that prioritize client-centered care and promote optimal outcomes.
* Maintain accurate, up-to-date clinical records, adhering to professional standards and regulatory requirements.
