The successful candidate will join an established Interdisciplinary CDNT, which reconfigured in 2014, providing quality, evidenced based and family-centred supports for children and young people aged 0-18 years, who present with complex needs associated with disability, within an interdisciplinary framework, to support their development to enable active participation in community life.The Social Worker will support children who present with complex needs associated with a disability on Kare's Children's Disability Network Team (CDNT) and will work as a key member of the interdisciplinary team providing a family centred quality service to children and their families in line with the National Programme for Progressing Disability Services for Children and Young people.
