Join Our Team as a Client Care Coordinator!
Location: Dublin North
Salary: Up to €38,000 (based on experience & qualifications)
Hours: Monday–Friday, 9:00am to 5:00pm/8:00am to 4:00pm with 1-2 weekend days per month, 2 evenings per month.

This role is perfect for someone who enjoys a mix of teamwork, problem-solving, people interaction, and a dash of hands-on care when needed .
What You’ll Do
* Coordinate and manage care services for a group of amazing clients.
* Liaise with clients and families, offering support and guidance with warmth and empathy.
* Create and maintain person-centred care plans, updating them as needs change.
* Mentor and support our Healthcare Assistants to deliver top-notch care.
* Keep care records up to date using our OneTouch Health system.
* Visit clients for reviews, assessments and provide hands-on care provision.
* Ensure everything we do meets our high standards and HSE/HIQA regulations.
What You’ll Need
* Experience in care coordination in a home care setting.
* QQI Level 5 Major Award in Healthcare Support (which must must Care Skills and Care of the Older Person) & a relevant qualification at QQI Level 6 or higher.
* Confident using Microsoft Office and care software (OneTouch Health experience is a bonus!).
* Great communication and people skills – you’ll be talking to clients, families and care staff daily.
* Excellent organisation and time management – every day is different!
* Full driving licence & access to a car (travel allowance provided).
* Eligibility to work in Ireland without requiring a work permit.
