Our organization is currently seeking a skilled and experienced Clinical Nurse Specialist (CNS) to fill the position of CNM2 Health and Chronic Disease Management. About the Role
The successful candidate will be responsible for providing specialist nursing care and advice to individuals with complex health needs, focusing on chronic disease management. They will work collaboratively with multidisciplinary teams to ensure seamless service delivery and promote person-centered care pathways.
* Develop and implement comprehensive individualized plans of care based on best evidence-based practice
* Provide expert clinical guidance and support to nurses and other healthcare professionals
* Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed
* Collaborate with colleagues across Primary and Secondary Care to deliver integrated care services
The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work autonomously and as part of a team. They must have a strong commitment to continuous learning and professional development, as well as experience in health and chronic disease management.
Key Responsibilities
The CNS/CNM2 in Health and Chronic Disease Management will:
* Participate in the development and implementation of evidence-based PPPGs and guidelines related to health and chronic disease management
* Monitor and evaluate the effectiveness of these guidelines and make recommendations for improvement
* Lead clinical governance initiatives and contribute to quality improvement projects within the organization
* Develop and maintain relationships with external stakeholders, including healthcare providers and community organizations
The successful candidate will be expected to demonstrate a high level of proficiency in IT skills, particularly in using Microsoft Office software. They must also be able to communicate effectively with individuals from diverse backgrounds and cultures.

Requirements
To be considered for this role, you must meet the following requirements:
* Be a registered nurse/midwife on the active Register of Nurses and Midwives held by An Bord Altranais and Cnimhseachais na hireann (Nursing and Midwifery Board of Ireland)
* Have a minimum of 2 years' post-registration full-time experience in health and chronic disease management
* Hold a minimum of 60 ECTS credits at NFQ Level 9 or higher in a relevant field
* Demonstrate continuing professional development (CPD) relevant to the specialist area
