Job Title: Advanced Practice Nurse
Job Summary:
We are seeking a highly skilled and experienced Advanced Practice Nurse to join our team at the UPMC Institute for Health in Mallow. The successful candidate will be responsible for delivering high-quality patient care, managing warfarin patients, conducting blood pressure monitoring, supporting Holter monitor services, and assisting in consultant-led clinics.
Key Responsibilities:
1. Deliver high-quality patient care and act as a preceptor to pre-registration students
2. Manage warfarin patients and conduct blood pressure monitoring
3. Support Holter monitor services and assist in consultant-led clinics
4. Develop and maintain effective communication and interpersonal relationships with all members of the multidisciplinary team
5. Identify and discuss nursing/patient needs with consultants/nursing team, multidisciplinary team, patients and carers
6. Accept responsibility and be accountable for the holistic care of a named group of patients on a shift basis
Requirements:
* Registered Nurse with the Nursing and Midwifery Board of Ireland (NMBI)
* Minimum of 3 years' post-registration clinical experience, ideally in primary care, ambulatory care, or hospital outpatient settings
* Experience in chronic disease management, anticoagulation, or women's health is highly desirable
