Project Worker Opportunity
Join our dynamic team as a Project Worker in the Own Front Door Project. This exciting opportunity will enable you to make a real difference in the lives of individuals and families experiencing homelessness.
The successful candidate will be responsible for conducting needs assessments, developing care plans, and providing support services to clients. This will involve working collaboratively with a range of stakeholders, including mental health professionals, social workers, and local authority representatives.
In this role, you will have the opportunity to work closely with service users to identify their housing and recovery goals, and to develop tailored support plans to achieve these objectives. You will also be responsible for monitoring client progress, identifying areas for improvement, and making recommendations for future support.

Key Responsibilities:
1. Conduct comprehensive needs assessments with service users to identify their housing and recovery goals.
2. Develop and implement tailored support plans to achieve client objectives.
3. Work collaboratively with a range of stakeholders to provide coordinated support services.
4. Monitor client progress and make recommendations for future support.
5. Identify areas for improvement and contribute to the development of new initiatives.
Requirements:
* 2 years post-qualification experience in a related field.
* Level 7 or above in Social Care.
* Excellent problem-solving skills.
* Good verbal and written communication skills.
* Full clean driving license and access to a car.
