Job Title: Care Coordinator

This is a fantastic opportunity to join our team as a Care Coordinator and make a real difference in the lives of our service users. As a Care Coordinator, you will play a key role in supporting individuals to reach their full potential, promoting independence and dignity, and enhancing their overall well-being.
Key Responsibilities:
1. Provide personal care and support to service users, including assisting with daily living tasks, medication management, and mobility aid use.
2. Assist service users with accessing community resources, social activities, and leisure pursuits.
3. Develop and maintain relationships with service users, families, and other stakeholders, ensuring open communication and collaboration.
4. Participate in goal-setting and planning with service users, focusing on promoting independence and self-advocacy.
5. Contribute to a safe and supportive environment, adhering to all relevant policies and procedures.
Requirements:
* QQI Level 5 in Healthcare or Childcare
* Legal right to work in Ireland without restriction
* Full drivers licence
* Relevant work experience desired
* Additional training will be provided
* Willing to undergo Garda Vetting and International Police Clearance (if lived outside of Ireland for more than 6 months after the age of 16)
