To promote and support independent living and community integration, we facilitate various educational, developmental, recreational, and occupational programmes and activities. These initiatives cater to the unique needs and capabilities of our service users, allowing them to reach their full potential.
Main Responsibilities
* To develop and deliver tailored programmes that address the specific needs of service users, as identified through Person Centred Planning. Record-keeping and maintenance are crucial aspects of this role.
* To serve as key workers, facilitating the Person Centred Planning process and identifying individual goals and required supports. Accurate record-keeping is vital in this capacity.
* To assist individuals and groups in accessing their community, thereby meeting identified needs. This involves coordinating activities and programmes, as well as performing other duties deemed necessary by the Programmes Supervisor / Community Services Manager.
Requirements
* A minimum QQI Level 5 qualification in Social Care or equivalent.
* Detailed knowledge of New Directions and community-based programmes, as well as intellectual disabilities and mental health issues.
* At least one year's experience working in a relevant setting, with a proven track record of facilitating and implementing person-centred plans.
