Responsibilities
* Be familiar with and adhere to all company policies and procedures.
* Use company IT systems as instructed and maintain data security in line with GDPR.
* Help people who have mental health conditions live more successful lives.
* Work as part of a specialist multidisciplinary team to provide a holistic approach to assist each patient.
* Understand the unique challenges of those with whom they work and provide appropriate support.
* Collaborate with team members on Initial Assessments, Individual Sessions and Group Sessions.
* Analyse living conditions and identify causes for concern or areas for improvement.
* Compile findings and analyses into reports for the team.
* Help patients understand their situations, set achievable goals, and support them to improve their lives.
* Work within legal frameworks (e.g., Mental Health Act 2001) and undertake continuing professional development as appropriate.

Qualifications & Experience
* Have a recognised qualification as a Social Worker with CORU registration
* 3 years minimum experience within a similar social care setting
