Job Summary
This role focuses on delivering high-quality patient care while collaborating with multidisciplinary teams. The Nurse Care Coordinator will work closely with children and their parents to provide education, support, and resources.
Key Responsibilities:
* Develop and implement individualized care plans in collaboration with healthcare professionals
* Provide education and training to families and healthcare staff on developmental dysplasia of the hip (DDH) service procedures and best practices
* Serve as a liaison between patients, families, and healthcare providers to ensure seamless communication and coordination
* Advocate for patients and their families to ensure timely access to specialized services and support
The Nurse Care Coordinator will also contribute to the development of educational programs and materials that enhance the quality of care delivered by multidisciplinary teams.
Essential Qualifications:
* Current registration as a nurse on the active Register of Nurses and Midwives held by An Bord Altranais and Cnimhseachais na hireann (Nursing and Midwifery Board of Ireland)
* Minimum 1 year post-registration full-time experience or aggregate 1 year full-time experience in relevant nursing specialty
* Completion of a post-registration program of study with Quality and Qualifications Ireland (QQI) Level 9 or higher award
* Continuing professional development (CPD) relevant to the specialist area or willingness to obtain required CPD
