Healthcare Direct is a leading provider of therapy services in Ireland, and we're currently seeking a passionate and dedicated Social Worker to join our team in Cahercalla Rehab Unit, Ennis, Co. Clare on a permanent basis. This is an exciting opportunity for someone with a strong background in social work who wants to make a difference in post-acute rehabilitation care.
About Healthcare Direct:
Healthcare Direct provides multidisciplinary interventions across Ireland, supporting various care settings and offering post-acute rehabilitation services. Our services integrate seamlessly with Mowlam Healthcare and the HSE, providing tailored care that enhances patient outcomes. With ongoing growth and development in integrated rehabilitation, we are looking to expand our team.
Key Responsibilities:
1. Patient Assessment: Assess patients' mental health, social, and financial needs to create comprehensive care plans.
2. Counselling: Offer emotional support and guidance to patients and families during times of illness, injury, or trauma.
3. Care Coordination: Collaborate with multidisciplinary healthcare teams to ensure the smooth execution of care plans, including discharge planning and rehabilitation.
4. Resource Referral: Connect patients and families with essential community resources like financial assistance, housing, and mental health services.
5. Crisis Intervention: Provide immediate and compassionate support during medical crises, including end-of-life care.
6. Advocacy: Ensure patients’ rights are respected, advocating for their needs within the healthcare system.
Ideal Candidate:
1. CORU Registration
2. Experience in social work with clinical experience in elderly care preferred (but not essential)
3. Strong written and verbal communication skills
4. Must have own transport, a clean driver’s license, and a smartphone
5. Passionate about providing patient-centered care in a dynamic, multidisciplinary environment
