We are seeking a skilled Programmes Facilitator to join our team. As a key worker, you will play a crucial role in facilitating the Person-Centred Planning process for service users.
Key Responsibilities:
* To be a key worker to service users and facilitate the Person-Centred Planning process
* To identify goals and supports required by individual service users and maintain records as required
* To develop and deliver person-centred programmes to meet the needs of service users
* To support individuals and groups in accessing their community to meet identified needs
You will work on your own initiative as a lone worker and liaise with relevant team members to ensure that service users receive the highest standard of service. You will also communicate with external agencies and families, maintaining the relevant records.
Requirements:
* A minimum QQI Level 5 in Social Care or equivalent
* A third-level graduate in Health & Social Care or equivalent is desirable
* Experience supporting adults with an intellectual disability and people enduring mental health issues
* Programme facilitation skills and client handling experience are advantages
The ideal candidate will be able to promote and support independence and community integration in their approach with the service user group. They will also comply with health and safety legislation and maintain the highest standards of safety, hygiene, and good housekeeping within the service.
Additional Requirements:
* Holding a full, clean manual driving license for at least two years
* Will to travel
This role requires a professional and respectful approach when interacting with service-users. The candidate should be willing to report any concerns to the supervisor or manager as appropriate.
