Location: Oldcastle, Co Meath; Part-time (3 days per week)
* Clinical Risk Grader Navigator (CRGN) with wound care experience needed for community nursing role.
* The role is Monday to Friday, with optional weekends.
* Hours: 9am – 5pm, Monday, Wednesday and Fridays.
* This is a temporary 3-month position with potential extension for the right candidate.
* A car and full clean driver’s license are required due to essential travel.

About the Role
We are seeking a skilled CRGN to join our team as a Clinical Risk Grader Navigator in the HSE Community Public Health service for Co Louth. This is a part-time position working 3 days per week.
In this role, you will work closely with healthcare professionals to assess patient risks and provide guidance on necessary interventions.
Key Responsibilities
* Conduct thorough risk assessments for patients in the community setting.
* Develop and implement individualized care plans to mitigate identified risks.
* Collaborate with multidisciplinary teams to ensure seamless patient care.
* Maintain accurate records of patient interactions and risk management strategies.
* Participate in regular team meetings to discuss patient progress and address emerging issues.
Requirements
* Minimum of [X] years of experience as a CRGN or similar role.
* Proven track record of providing high-quality patient care and risk management services.
* Demonstrated ability to work effectively in a fast-paced community setting.
