Job Title: Community Support Worker
We are a voluntary organisation providing services and support for people with disabilities and their families for many years.
Job Description:
As a Community Support Worker, you will promote and support independent living and community integration, personal development and work opportunities for service users. Your role will be to assist the Community Services Manager in the development, implementation and delivery of quality customer-focused and cost-effective community services to service users at an appropriate time and setting.
* Provision of individual programmes and activities to meet the needs, choices and abilities of service-users.
* Active contribution towards the involvement of RehabCare in the expansion of this service.
You will also provide personal care if and when required, dependent on the individualised needs of those who use the service.
Key Responsibilities:
* To plan, develop, implement, monitor and review appropriate educational, recreational and occupational programs within the community.
* Liaise with families, the multidisciplinary team and key community-based personnel in the pursuit of accessible community activities for service users.
* Document information, update records, and complete necessary paperwork for communication and accountability purposes.
Required Skills and Qualifications:
* Previous experience of working in the community.
* Experience of working 1:1 in the community.
* Experience of promoting self-advocacy.
* Experience in the provision of personal care.
