CNM 2 is responsible for providing the highest standards of Person Centred Care in line HIQA standard, their job description, relevant legislations and guidelines. She/he has responsibility for the supervision of the Nurses and Healthcare Assistants and all other staff with a role in her designated area. The post holder must always act to the highest standard of professionalism and in line with the Core Values.Main Duties and Responsibilities:Service Management and Development· Undertake the role and responsibilities of the Clinical Nurse Manager 2 (CNM2) in line with the Health Act 2007, related regulations, all other legislative requirements ie. Health and Safety, Fire, Employment, Environmental, General Data Protection Regulation and the Health Information and Quality Authority (HIQA) Standards, An Bord Altranais.· Participate the organisation, management and operations of the Nursing Home under direction of the Asst/ Director of Nursing.· Participate in the development, training and supervision of staff.· Liaise with external organisations in order to promote and maintain good working relationships and high standards of care and service for residents.· Responsible for all aspects of risk management and resident safety and protection.· To ensure safe use and care of all equipment in your area of responsibility.· Implement stock control and have knowledge of your specialist equipment ie air mattresses /pumps, hoists, low low beds· Keep abreast of recent trends in practice development and be able to lead change and improvement initiatives.· Ensure that the provision of a high standard of Person Centred Care to the resident and families in the nursing home is consistent with the mission, vision, values and strategic plan of Nazareth House Nursing Home.· Oversee the delivery of the highest quality resident care together with all members of the multidisciplinary teams, nursing, health care care, cleaning, dietary, laundry, maintenance, admin and finance staff.· To effectively lead all staff in striving to achieve the mission and core values of the organisation.· To ensure delivery of a service that will contribute to the well being of residents through effective leadership, motivation and management of staff, volunteers and resources.· To lead the development of policies, procedures, guidelines and team practice to ensure that the care provided meets residents needs in line with National Standards and best practice as laid down by HIQA and other government bodies.· To identify and manage gaps in service provision and opportunities, to learn from audits, incidents, accidents, complaints and lead change and improvement initiatives and practice developments within the service.· To ensure that all quality reports and bed management lists are up to date.· To participate in service planning in association with the Asst/ Director of Nursing.· To ensure regular service reviews and audits are undertaken and implementation/evaluation of, learning from and improvement plans implemented as a result.· To ensure that all accidents, incidents, complaints, near misses and safety concerns are documented and reported to Asst/ Director of Nursing· To adhere to An Bord Altranais professional standards including “Professional Guidance for Nurses Working with Older People” (An Bord Altranais 2009).· To lead, mentor ongoing team building and development to ensure that the Nursing Home provides excellent care.· You are required to treat staff in a professional manner, treating all staff equally and fairly.· Showing impeccable standards of confidentiality in respect of all areas of your practice and instilling those standards in your team.· To ensure that support and supervision is provided to a multidisciplinary team of staff and volunteers, and that regular performance and personal development appraisals are undertaken with all staff.· To oversee staff communication and ensure that regular team meetings are held and recorded.· To ensure correct and pertinent information regarding resident needs and care is communicated to all staff to ensure appropriate continuity of care is provided· Developing rosters in a timely efficient and fair manner to ensure that there is adequate cover in your designated area through ongoing operation of a comprehensive staff rota that takes account of residents’ dependency needs.· As the lead nurse manager in your designated area you will be on call for all areas at week-end in your turn, you may be required to cover a nursing shift if there is absolutely no alternative· Monitor, audit poor attendance. Deal with under the appropriate procedures in conjunction with the Asst/ Director of Nursing.· To ensure that all new staff receive appropriate induction to their roles and work within the Nursing Home, including knowledge of HIQA standards and the related policies, procedures and guidelines as applied in the Nursing Home, to meet the needs of the residents.· To identify and manage through the appropriate procedures any shortcomings in the performance of new staff and report to the Director of Nursing in a timely manner.· To undertake yearly performance for all staff under your direct leadership, implement improvement plans and evaluate.Management of Care· To ensure that Person Centred and Quality Care is provided in line with Best Evidence Based Practice, Ethos and Core Values of the Nursing Home’s policies and procedures, and in accordance with National Standards and HIQA regulations.· To manage resident referrals including assessment of client and care needs and ensure continuity of care for all residents including the development,· Development, implementation and regular review of person centred care plans as per HIQA Standards to ensure that the availability of appropriate client centred activities are provided, documented and evaluated so that clients health and well being is promoted.· To ensure that a comprehensive, person centred approach is taken to risk assessment and risk management in all areas and that all assessments and care plans are maintained in line with HIQA requirements.· To ensure that the needs of all residents’ families are met and supported as appropriate.· To be vigilant at all times and promote and instill a culture of vigilance for resident safety, protection and rights.· Ensure that all your team are clearly knowledgeable on the rights and entitlements of resident to a person centred quality respectful, safe, and dignified care.· To ensure that Health & Safety and Hygiene standards are treated as priorities and that all related policies and procedures are enforced in compliance with statutory requirements.Administration & General Management· To ensure that communication with Director of Nursing is timely, accurate and comprehensive.· To ensure that data and information on activities (including statistics and other relevant data) are recorded using the current system in use by organisation and to report to Asst/Director of Nursing.· To strictly adhere to the policy of confidentiality and to ensure a culture of confidentiality is promoted in the home and all the team is familiar with the requirementsThis is a senior position and the post holder is expected not to require clinical supervision based on their advanced level of clinical skills and expertise.PERSON SPECIFICATIONKnowledge/Experience:· RGN with Full ABA (An Bord Altranais) registration.· Evidence of continuous professional development.· Minimum of 3 years out of the last 6 years in an Aged care setting at management level in the health and social care area.· Post registration management qualification in health or related field.· Excellent organisational and communication skills with fluent English.· At least 2 years management experience, ideally in a relevant setting.· An understanding of Dementia including: current best practice in dementia care, the issues facing people living with Dementia and their carers/families and the services available.· A post registration qualification in nursing the older person is desirable.· An understanding and experience of Quality and Audit.· A thorough knowledge and understanding of the legislation governing services for older people and people with dementia including HIQA standards and regulations and their practical applications.· An understanding of other legislation relevant to the role e.g. Health & Safety, Employment legislation etc.· Service development and service planning experience is desirable.· Ability to develop relationships and liaise with external agencies including primary care and relevant specialist services.· Strong leadership skills, able to motivate, lead and support a team in an organisation that aims for best practice and care.· Commitment and ability to lead a Person Centred Service with Evidence of Continuous Quality Improvement and Innovation.- Ability to supervise, manage and motivate a team of nurses and other staff- Ability to audit service, plan and manage change- Excellent organisational skills in the area of general administration, resource allocations etc.- Financial awareness and ability to operate service within a budget.- Ability to handle effectively challenging deadlines and multiple tasks· Excellent communication (verbal and written) and interpersonal skills· Strong negotiation and influencing skills· Comprehensive understanding of the Irish health system and structures· Experienced computer user with strong MS office skills
