Purpose of the Position-
The Day Care Nurse Manager is responsible for providing best practice dementia specific care to persons living with dementia in the daycare setting. The successful candidate will manage and co-ordinate person centered care and resources in the daycare in an efficient and effective manner to people living with dementia (PlwD).
You will have responsibility for the supervision of Day Care staff and will work closely with other members of staff to ensure they are cognisant of other services which the PlwD/family may be receiving so that they can help ensure that all services are delivered in an integrated seamless manner.
The successful candidate will be responsible for health and safety and administration of the Day Care service while providing and promoting a model of care which is person centered, community focused and has an appreciation of the needs of the family carer.
· Appointment is subject to a satisfactory outcome from the Garda Vetting Process and receipt of suitable references.
· You will be employed as a Day Care Nurse Manager with Western Alzheimers
· You will have a reporting relationship to the Regional Manager and in their absence the CEO.
· You will be required to be flexible in this position and must be prepared to undertake such other duties as may be assigned to you by the Regional Manager, from time to time, including training.
Main Duties & Responsibilities-
· You will be required to adhere to and comply with all in-house rules, regulations, policies, procedures, and codes of practice in force from time to time.
· You are also required to implement policies to safeguard the safety and welfare of staff, PlwD and other persons onsite, including full compliance with all relevant legislation and in particular the Health, Safety & Welfare at Work Act 2005, as amended from time to time by safety legislation.
· You will be responsible for the day-to-day running and staffing of the daycare and all operational detail.
· You will be responsible for ensuring that the service is promoted amongst other healthcare professionals, who will ultimately refer to the service.
· You will be required to assess the referrals and make appropriate decisions.
· You will be required to ensure that as many people living with dementia as possible receive day care service by ensuring that Daycare operates at maximum capacity.
· You will be responsible for the care support plan for each client of the daycare.
· You will foster and support a 'can-do' attitude in supporting the person living with dementia.
· You will support clients to remain as active as possible and as independent as possible.
· You will support Western Alzheimer's ethos of generating awareness of dementia and making your community a dementia friendly environment.
· You will work with the Western Alzheimers team in Co. Galway and elsewhere when required.
· You will ensure that the daycare environment is risk free and that hygiene standards are maintained at all times.
· Order meals for those attending and ensure staff have the required HACCP training necessary to monitor food quality.
· You will be cost conscious to ensure that resources are maximised.
Management-
● Co-ordinate recruitment and ensure that all employees receive appropriate induction and their ongoing training needs are periodically reviewed and actioned.
● Ensure fair and consistent application of all Western Alzheimers policies and procedures as applicable to the Day Care service.
● Conduct regular performance reviews with all staff and deal with any breaches of policy, disciplinary or grievance matters in a timely manner.
● Liaise and take advice from Regional Manager where necessary.
● Hold regular individual and group staff meetings to discuss performance, service enhancements, client welfare and relevant operational or procedural matters.
● Assess, identify, and manage the needs of potential and existing clients.
● Carry out and input into the assessment (initial and ongoing) of clients and decide whether Day Care, or Home Care is/are the most appropriate service(s) to offer the client.
● Where the person with dementia out grows the service, you will discuss a discharge plan with the family involved, providing adequate time and discussion for a smooth transition.
● Reconcile daycare fees and lodgments to finance manager.
● Provide a monthly stats report to the Regional Manager and an annual register of all persons with dementia who availed of the service throughout the year.
