Job Description
As a Support Coordinator, you will provide frontline services to individuals aged 60 and over. Your role will involve assessing needs, connecting with partners, and coordinating services to ensure a comprehensive approach.
You will devise personalized support plans to address challenges and enhance the quality of life for older people. This includes providing access to services, advocating for their needs, and streamlining service delivery.
You will establish strong referral relationships with agencies in the designated area, fostering collaboration and access to support services. You will also develop efficient referral pathways for ALONE's Support Coordination services.
Key responsibilities include:
1. Providing support coordination by devising personalized support plans
2. Providing access to services while coordinating and enabling older people to access other services
3. Advocating for older people's needs within health, social care, and welfare services
4. Establishing and nurturing strong referral relationships with agencies
5. Developing and maintaining efficient referral pathways
