This is a fantastic opportunity to play a pivotal role in overseeing and enhancing their HSE rehabilitation bed project. Job Type: Full-time, Permanent Location: Galway, Ireland Your Mission: As the Nursing Home Project Coordinator, you will be the driving force behind the seamless delivery of rehabilitative services, ensuring exceptional patient journeys from admission to discharge. You will be instrumental in fostering collaboration, maintaining operational efficiency, and ensuring the successful execution of this vital project. Your Responsibilities Will Include: Taking full ownership of the HSE rehabilitation bed project, managing every stage from patient admission through to discharge. Overseeing the daily operations of the rehabilitative services to guarantee their efficient and effective delivery within the assisted living facility. Building and nurturing strong working relationships with the multidisciplinary team (MDT), including nursing, medical professionals, physiotherapy, occupational therapy, social work, and HSE representatives. Serving as the central point of contact for internal teams, the HSE, and external partners, facilitating clear and effective communication and collaboration. Developing, monitoring, and reporting on key performance indicators (KPIs) on a weekly basis to track patient outcomes, performance, and adherence to contract terms. Identifying trends and deviations in service delivery and proactively implementing solutions to address any issues or inefficiencies. Collaborating with financial, legal, and operational departments to track and manage contract deliverables, deadlines, and service-level agreements. What You Need to Succeed: Active registration with the Nursing and Midwifery Board of Ireland (NMBI). A knack for effective problem-solving and a meticulous attention to detail. Exceptional time management, prioritisation, and multitasking abilities. A dependable, adaptable, diligent, and cheerful disposition with a strong work ethic. The ability to work autonomously and take initiative. Tech-savviness with prior experience using software systems such as SAP, Microsoft Projects, Teamwork, etc. Strong IT knowledge is highly desirable. Excellent interpersonal skills and a commitment to providing outstanding customer service. A willingness to take on additional responsibilities when required.
