Details of ServiceDamien House Regional Services (DHRS): DHRS provide community based residential care and day services for Adults with moderate/severe/profound levels of intellectual disability, behaviours of concern and physical, mental health and medical conditions. The centres are governed by a CNM2/CNM1/Social Care Leader structure. The local Interdisciplinary Team includes a Clinical Nurse Specialist in Behaviour Support and Senior Clinical Psychologist.DHRS adopts a biopsychosocial model of care and provide person centred care in community residential homes and day centres. Each community home is directly managed by a CNM2/CNM1/Social Care Leader and overall management of the service is the responsibility of the Regional Social Care Manager and the Director of Nursing.DHRS comprises of 4 centres in CHO5 – Damien House Services, Clonmel, Co.Tipperary; Avila, Fethard, Co.Tipperary; Re Nua, Cashel, Co.Tipperary and Leeside, Callan, Co. Kilkenny.Purpose of the PostThe role of the Social Care Worker is to work as part of a multidisciplinary team in the delivery of care to the Resident under the supervision of the Clinical Nurse Manager (CNM2/CNM1)/Social Care Leader.
