Speech Language Pathology LicenseAHA Basic Health Care Life Support HCP CertificationResponsibilitiesThe Speech Pathologist is responsible for providing professional therapy to patients in their homes, including evaluation, development, and implementation of individualized, goal-oriented therapy programs in accordance with organizational policies and external guidelines.
Complete patient evaluations within the specified timeframe, ensuring assessments are holistic and comprehensive.Develop individualized treatment plans based on evaluation findings, focusing on functional, measurable short- and long-term goals.
Prepare EMR documentation such as progress notes, reassessments, and discharge summaries, ensuring they reflect skilled care and monitor patient progress.Ensure documentation is accurate, legible, timely, and supports the need for continued skilled care.Follow physician's plan of care regarding service frequency and duration.Coordinate effectively with physicians, MCP, and other healthcare team members, ensuring appropriate referrals and resource utilization.Educate patients and/or caregivers, providing written home programs and comprehensive education tailored to their needs.Build rapport and mutual respect with patients to foster optimal environments for goal achievement.
Implement treatment plans using appropriate clinical skills and techniques.
Consistently meet productivity requirements.Participate in discharge planning, addressing problem areas appropriately.Complete scheduling accurately and promptly, updating systems as needed.Respond to voicemails and messages in a timely manner.Document communication with healthcare team members using secure messaging systems.Ensure discharge summaries accurately reflect patient status, outcomes, and recommendations for further care.Perform other duties or projects as assigned, demonstrating flexibility in responsibilities.Adhere to HMH's organizational standards and behavioral competencies.Lift and push/pull a minimum of 50 lbs., and stand for at least 8 hours daily.
