Social Worker – Supporting Adults with an Intellectual Disability

Role
* Carry a case‑load, including a greater complexity of individual/family need.
* Ensure that the rights of the people we support and their families are respected.
* Inform the person supported and their family of their rights, entitlements, benefits and services.
* Guide the people supported in all areas of Advocacy (self‑advocacy and group advocacy).
* Support monthly advocacy sessions with preparation of discussion topics and materials.
* Together with the people supported, identify areas of concern and help to put plans in place to overcome lived challenges, both on a local, regional and national level.
* Identify unmet needs and make recommendations accordingly.
* Work with staff, managers and other clinicians in devising and implementing care plans for the people supported and support plans for families.
* Participate in multi‑disciplinary team meetings and work in co‑operation and collaboration with team members.
* Participate in case conferences, case reviews, team meetings and other meetings as required.
* Support Designated Officers for safeguarding vulnerable adults at risk of abuse.
* Participate in the admissions and discharge processes as required.
Contribute to DSAMT/ funding applications / business case submissions to HSE.
* Ensure notes are documented and reports furnished as appropriate.
* Participate in planning, policy formation and research as required.
* Ensure the highest standards of professional practice are maintained in the Social Work Department in line with current legislation and acting in accordance with organisational guidelines and procedures.
* Carry out such other duties as may be assigned from time to time.

Professional Qualifications & Experience
Essential
* The person appointed will hold a professional social work qualification in Social Work (NQSW/CQSW) or equivalent qualification.
* Applicants must be registered with or in the process of registering with CORU.
* Have at least 1 year’s relevant professional experience in Social Work.
* Experience of working in a multi-disciplinary environment.
* Competent and confident IT Skills.
* Full Clean Driving Licence.
Desirable
* Experience in working with people with an Intellectual Disability.
