Role Purpose
The role is responsible for providing a service which optimises quality and continuity of care for patients/clients, providing an informed resource for nurses, doctors and other allied health professionals, and providing a programme of education and training for multi-disciplinary staff that benefits patients/clients.
The position requires a strategic approach to the development of services and structures, embracing continuous quality improvement and the management of changes necessary to achieve organisational objectives.
Qualifications / Experience
Be registered in the general division of the Register of Nurses maintained by NMBI
Hold a Diploma in Palliative Care or equivalent qualification.
Have a minimum of 3 years post registration experience in Oncology or General Nursing.

Key Responsibilities
Operations
The Clinical Nurse Specialist will advise and treat all patients requiring palliative care.
The Clinical Nurse Specialist will educate patients and their carer's, nursing and allied health professional colleagues regarding palliative care.
The Clinical Nurse Specialist will act as an expert in providing information and assistance to nursing and allied health colleagues to their request for advice and support on palliative care.
The Clinical Nurse Specialist will monitor and critique current nursing research in relation to oncology care and will lead in the initiation and implementation of best practice in nursing patients with cancer.
The CNS work must have a strong patient focus whereby the speciality defines itself as nursing or midwifery and subscribes to the overall purpose, functions and ethical standards of nursing or midwifery.
The clinical practice role may be divided into direct and indirect care.
Direct care comprises the assessment, planning, delivery and evaluation of care to patients and their families.
Indirect care relates to activities that influence others in their provision of direct care.
The CNS role involves communication, negotiation and representation of the patient/client values and decisions in collaboration with other health care workers and community resource providers.
The CNS remit for education and training consists of structured and impromptu educational opportunities to facilitate staff development and patient/client education.
Each CNS in tandem with his/her line manager is responsible for his/her continuing professional development, including participation in formal and informal educational activities, thereby ensuring sustained clinical credibility among nursing/midwifery, medical and paramedical colleagues.
Audit of current nursing/midwifery practice and evaluation of improvements in the quality of patient/client care are essential requirements of the CNS role.
The CNS must keep up to date with relevant current research to ensure evidence based practice and research utilisation.
The CNS must contribute to nursing/midwifery research, which is relevant to his/her particular area of practice.
Any outcomes of audit and/or research should contribute to the next service plan.
Inter and intra-disciplinary consultations, across sites and services are recognised as key functions of the clinical nurse/midwife specialist.
This consultative role also contributes to improved patient/client management.
Maintain throughout the hospital an awareness of the mission and values of the hospital in relation to patient care.
Demonstrate behaviour consistent with the values of the hospital.
Maintain the privacy of the patient in relation to all hospital activities.
Act as a specialist palliative care resource for patients with advanced disease and their families referred to the palliative care team.
Act as a specialist palliative care resource for health care professionals in the hospital.
Be responsible for own caseload of patients, visit them regularly and initiate changes in management to improve symptom control and psychological well-being.
Anticipate the many and varied palliative care needs of patients and families, devise and implement strategies to meet these needs and evaluate their effectiveness.
Establish and maintain good working relationships with nursing staff and all other disciplines involved in care to provide good quality palliative care for patients and their families both within the hospital and community services.
Act as a patient advocate within the multi-disciplinary team.
Take responsibility for making often complex and difficult decisions in the area of pain and symptom management.
Address ethical issues and family dynamics.
Provide a supportive counselling service for patients, assessing their psychosocial and spiritual needs and refer on to more formal counselling services as necessary, be aware of the impact the illness may be having on the patients and their families.
Refer patients and families to specialist palliative care teams in the community or to inpatient palliative care units as necessary.
Ensure that the dignity of the patient is ensured and maintained at all times and having regard to the ethics and philosophy of the hospital.
Ensure patient confidentiality is maintained at all times.
Effectively manage the palliative care requirements of a caseload of patients.
Establish and maintain clear referral lines.
Participate actively in the development and strategic planning of the palliative care service.
Represent the team at local, national and international meetings as required.
Identify the need for implementing and evaluate innovation in specialist palliative care in the hospital.
Maintain clear and accurate records, including statistical data relevant to all aspects of patient care and submit activity data as required.
Promote a safe clinical environment with due regard to health and safety and risk management strategies.
Promote a patient centred culture of openness and responsiveness to positive and constructive patient feedback and ensuring patient dignity, respect and choice is maintained in their care.
Learning and Education
Act as a role model in the clinical area.
Promote the role of specialist palliative care services amongst appropriate health care professionals throughout the hospital.
Identify the palliative care education and training needs of staff and initiate strategies to meet these needs both at a formal and informal level.
Liaise with the relevant education departments and participate in teaching on various aspects of pre and post registration nurses and allied healthcare professionals.
Attend study days and conferences in order to update knowledge in the field of palliative care and related subjects.
Keep abreast of new developments in palliative care through published specialist palliative care literature.
Promote and disseminate palliative care research.
Set standards to offer a quality service.
Act as a nursing research resource in palliative care for nursing staff throughout the hospital.
Initiate and participate in research projects being undertaken by the team once the team is established.
Advise on the implementation of appropriate research findings.
Quality Improvement and Risk Management
Ensure that there is compliance with all relevant Hermitage Medical Clinic guidelines, policies, procedures and relevant legislation and regulatory requirements.
This includes responsibility for dealing with regulatory or professional bodies and inspections.
Encourage continuous review and evaluation of policies, guidelines and existing practices through regular audit programmes and review engaging with the multi-disciplinary team where required.
Promote a culture of continuous quality improvement across the department involving the team in continuous improvement initiatives and encouraging innovation.
Set and monitor core objectives, standards and key performance indicators for the service and monitor performance against these standards through internal and (where applicable) external audit.
Lead out on the requirements of the hospitals accreditation process.
Participate fully in the requirement of the hospital's risk management programme.
Promote the delivery of a high standard of care to all patients.
Encourage evidence-based practice and assist in the dissemination and implementation of research findings.
Work with members of the Multidisciplinary team in devising Standard Operating Procedures for the development of the Unit.
Provide clinical nursing leadership in proactively addressing ethical and quality of service issues with multidisciplinary team members and hospital management.
Develop a culture of safety, promoting positive reporting of incidents and near misses; investigate and take remedial action on incidents or near misses involving patients and staff according to hospital policies, protocols and guidelines.
