Job Objectives We are seeking a Community Mental Health Nurse to join our dynamic team at Nua Healthcare, delivering person-centred care and supporting individuals to achieve positive mental health outcomes within the community. This role will include the following on a day-to-day basis: Live our Mission, Vision and Values. Provide person-centred, recovery-focused mental health nursing care to individuals within the community setting. Conduct comprehensive mental health, risk and need assessments, contributing to the development, implementation and regular review of individualised care, recovery plans and relapse prevention plans. Carry out community visits and home assessments where appropriate to support individuals in their own environments. Deliver nursing interventions, including monitoring mental health status, medication administration and management, and supporting adherence to treatment plans. Provide early intervention and relapse prevention support, identifying warning signs and supporting service users to manage their mental health proactively. Work collaboratively as part of a multidisciplinary team, including psychiatrists, psychologists, social workers, occupational therapists and other healthcare professionals. Identify, assess and escalate safeguarding concerns, risks or changes in a service user's mental health presentation in accordance with organisational and statutory procedures Maintain accurate, timely and confidential clinical documentation and reports in line with professional standards and organisational policies. Supervise and mentor Nursing staff within the Mental Health Centre Working with GPs, psychiatrists, social workers, housing, and voluntary services to ensure coordinated care. Participate in case reviews, clinical supervision, multidisciplinary meetings and service development initiatives. Promote mental health awareness, recovery principles and social inclusion within the community. Ensure compliance with relevant legislation, professional standards and organisational policies, including the Mental Health Act 2001 and the Mental Health (Amendment) Act 2018. Engage in continuing professional development (CPD) to maintain and enhance professional knowledge, skills and competence. Where appropriate, support the Consultant Psychiatrist in the co-ordination of admission to acute inpatient services through liaison with those services, advocating for the persons' need and direct correspondence with emergency services. Act as a lead clinician in the management of an acute mental health deterioration or crisis Collaborate with the regional nursing team, GPs, practice nurses in the assessment, care planning, management and review of physical health concerns. Supporting the Consultant Psychiatrist and other members of the MDT in assessing capacity and care planning around capacity concerns. Skills Requirement Essential Criteria Be registered as a Registered Psychiatric Nurse (RPN) with the Nursing and Midwifery Board of Ireland (NMBI). Have 3-5 years relevant experience in community mental health services or a related mental health setting. Desirable Criteria: Postgraduate in Community Mental Health Nursing. Previous experience working as part of a home-based treatment team, in psychiatric liaison or an acute psychiatric inpatient service. Knowledge: Knowledge of standards and legislation relevant to the area. Skills: Excellent overall demeanour. Effective verbal and written communication and IT skills. Good aptitude for learning. Good problem-solving skills. Good planning and decision-making skills. Effective verbal and written communication. Ability to work to deadlines under pressure. Ability to empathise with and understand the needs of others. Ability to work on own initiative. Reliability and flexibility. Self-motivated/self-starter team player.
