Job Overview
We are seeking a dedicated and compassionate Social Care Worker to join our organisation in delivering person-centred care and support to individuals with intellectual disabilities. This role is ideal for those who share our passion for making a positive impact on people's lives.
Key Responsibilities:
* Provide residential and respite care, supporting daily living, personal care, and household routines.
* Act as a key worker, developing and implementing individualised care plans that promote independence, dignity, and inclusion.
* Support individuals with complex needs and behaviours using evidence-based approaches that ensure their physical and emotional well-being.
* Administer and document medications in line with established policies and procedures.
* Foster meaningful relationships with service users, families, and multidisciplinary teams through effective communication and collaboration.
* Facilitate community-based activities that promote social inclusion and enhance the quality of life for individuals we support.
* Maintain accurate records and contribute to service planning and development through continuous assessment and evaluation.
* Provide guidance to care assistants and act as Person in Charge when required, ensuring seamless delivery of care services.
* Work a flexible rota, including weekends, sleep-ins, and night shifts as needed, to ensure continuity of care services.
* Drive service vehicles or your own vehicle for work-related duties, adhering to all relevant safety protocols.
Candidate Requirements:
* Essential qualification: Level 7 degree in Social Care or a related discipline.
* CORU registration (or in process).
* Experience supporting individuals with intellectual disabilities or in a similar care setting.
* Full driving licence and access to a vehicle.
* Strong communication, teamwork, and problem-solving skills.
* A motivated, empathetic, and person-centred approach to care provision.
