The Homecare Project requires a project worker to support tenants under the programme. The Good Shepherd Centre in conjunction with Mental Health Service Area 5 Kilkenny and in partnership with Kilkenny Local Authority will provide homecare support and a recovery orientated tenancy sustainment programme.

Venue: Tar Isteach Housing, Ormonde House, Barrack St, Kilkenny. Report: Team Leader Homecare.

The project aims to promote independent living in the community for service users with mental health needs, focusing on tenancy sustainment and recovery-oriented supports.

Funding is sourced from the Kilkenny HSE Mental Health Services and the Good Shepherd Centre to employ a project worker through the Grant Aid process.

Secure tenancies with support are recognised as the way forward for many of those with complex needs.


Main Roles And Responsibilities

* Work within the framework of the overall objectives and Policies of the Good Shepherd Centre.
* Ascertain and support service users' preferences in relation to fuller participation in their local and wider communities.
* This contract is a specified purpose contract. Subject to ongoing HSE Statutory Funding Support.
* Ensure support for tenants in accessing recovery-focused training, education and employment opportunities.
* Work as part of a multi-disciplinary team and service.
* Maintain a caseload of clients and offer ongoing support until cases are handed over to the appropriate support services.
* Supervise in conjunction with the manager the health care workers' role and duties.
* Accompany and provide support to clients with appointments to other services, where appropriate.
* Be responsible for keeping up to date casework records, daily logs and written assessments.
* Work in close liaison and cooperation with other teams across mental health and community-based services, including statutory and voluntary agencies as deemed appropriate.
* Adhere to the service model and standards of best practice.
* Meet regularly for supervision with your assigned Supervisor.
* Attend all nominated training by your line manager.
* Supervise students or volunteers as requested by assigned supervisor or senior manager.
* Work closely with the homecare support worker in the coordination and implementation of the homecare support care plan.
* Work a flexible roster over 5 days and occasional evenings to meet the service objectives.
* Implement existing policies and procedures and contribute to the development of new ones.
* Provide an on-call role within the organisation on a rostered basis.
* Plan, implement and evaluate daily activities.
* Undertake other duties as reasonably assigned from time to time in consultation with the Team Leader.
* Be vigilant to health, safety and welfare risks and report concerns.
* Engage with identified service users to facilitate defining and attaining housing and recovery goals using a strengths-based approach.
* Conduct needs assessments to inform agreed care plans and discuss how the service user would like to be supported.
* Assist service users to navigate Local Authority processes.
* Support service users in developing life skills to foster independence and autonomy.
* Communicate with the service user’s treating team to review needs and care plans.
* Facilitate peer support where appropriate.
* Support tenancy sustainment and risk management planning as required.
* Support the service user to self-advocate or advocate on their behalf when agreed.
* Case manage referrals to optimise tenancy sustainment and independent living.
* Promote the service user’s safety, health and wellbeing.


Essential Criteria

* 2-year post qualification experience in a related field
* Level 7 or above in social care
* Full clean driving license and access to a car
* Good verbal and written skills
* Excellent problem-solving skills
