As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.
BAYADA Senior Living offers a comprehensive continuum of care, including traditional home care under Medicare Part A and outpatient therapy under Medicare Part B. Our program emphasizes exceptional clinical care and relationship management, with BAYADA clinicians becoming true healthcare partners in senior living communities such as assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities for clients with intellectual disabilities.
Responsibilities for a PT - Physical Therapist include:
* Make home visits to clients in designated geographic territories.
* Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
* Implement protective interventions to prevent wounds and promote wound healing using evidence-based dressings and topical agents.
* Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and balance interventions.
* Assess and revise care plans regularly, collaborating with nursing in multidisciplinary care planning.
* Educate clients, family members, and caregivers on rehabilitative care and activities to promote health, safety, and independence.
* Document observations, interventions, and evaluations accurately using a touch pad tablet.
Qualifications for a PT - Physical Therapist include:
* Current NJ Physical Therapist (PT) license.
* Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
* Ability to work independently and manage time effectively.
* Strong interpersonal, organizational, and problem-solving skills.
* Solid computer skills; prior experience with electronic medical records (EMR) preferred.
