The purpose of the role is also to provide assessment and support to the individual through the delivery of appropriate treatment and recovery orientated interventions to support care in the community working towards reduction in the need for acute hospitalisation. This will include: domiciliary visits, family support and liaison with other services.These posts will support National Mental Health Clinical programme delivery. They will involve the staff members undertaking assessments, initiating a range of evidenced based recovery orientated clinical interventions and linking closely with a range of statutory services in secondary and primary care services and with voluntary services in the local community. They will be expected to work as a member of a community mental health team, undertaking standardised assessments and management along with assertive follow up involving families/carers as appropriate.
